Lindenhof Group: For Managed Care Patients - More Efficient and Safer Thanks to Automated Data Exchange

For over three and a half years, the practice network mediX bern and the Lindenhof Group have been cooperating based on shared values and treatment philosophies. One of the cooperation goals is to promote coordination and digital integration at the interface between outpatient and inpatient care. A constant companion on this journey is the ongoing digitalization in healthcare, which increases efficiency and improves the safety of patients. As the largest doctor-owned practice network in the Canton of Bern, mediX bern significantly supports the development and introduction of the digital platform 'Heureka'. This platform enables direct digital data exchange between various healthcare providers. It was developed by Healthinal AG. Based on the 'Heureka Platform', the product 'Smart Managed Care' was developed with the following goal: Direct digital access to the medical history in primary care improves the safety of patients, while simultaneously reducing administrative efforts between service providers in hospitals and primary care practices.

As the first hospital, the Lindenhof Group is testing the application 'Smart Managed Care HOSPITAL'. This allows for direct access to patient master data and medical patient data in the primary care practice information system. The pilot project involved the Bern Emergency Center at Lindenhof Hospital and two large mediX bern partner practices. Through the structured interface, the staff of the Bern Emergency Center, thanks to the 'Heureka' technology, has direct insight into the current medication list of the primary care medical history. This occurs without waiting times and is independent of the opening hours or holiday absences of the primary care practices. Managed Care patients, who have concluded an insurance product with mediX bern through their health insurance, benefit from this rapid availability of information. Throughout the 3-month pilot project with the Bern Emergency Center of the Lindenhof Group, consistently positive experiences were gathered.

Already 59 mediX bern partner practices are connected to the 'Heureka Platform'. This direct digital networking between these practices and the Lindenhof Group can be further expanded this year. The outpatient affiliates of the Lindenhof Group, Localmed & City Notfall, are also connected.

The data exchange will not be limited to the medication list: soon, diagnosis lists and laboratory data will also be able to be exchanged automatically. With this innovative approach and the automated data exchange between family doctor practices and the hospital group, mediX bern and the Lindenhof Group are taking on a pioneering role in Swiss healthcare.
